It's simple to segment your video into chapters. In your description, simply add a timestamp for the start of the chapter and the title of that chapter.
For example,
0:00 Introduction
0:47 Chapter 1: Narbacular Drop
1:56 Chapter 2: Early Portal
5:48 Chapter 2.5: The Creation of GLaDOS
8:33 Chapter 3: From Early to Final Portal
11:55 Chapter 3.5: Unused Assets
13:25 Chapter 4: Conclusion and Portal's Legacy
Any text after the timestamp (i.e. 0:47) will be used as the title.
